Housing Stabilization Services is a valuable resource available to individuals with disabilities and seniors covered by Medical Assistance. This service is designed to assist with finding and maintaining suitable housing arrangements. Referral to this service can be made by a waiver case manager, and in the case of those without a waiver, a professional statement of need and a housing-focused person-centered plan will be required. Such a plan can be created by a targeted case manager, care coordinator, or housing consultant. Superior Support Services will enroll the individual with DHS and arrange an intake meeting with a Housing Stabilization team member once services have been approved.
